Rachael has been in the healthcare field since 2007 and has a wide range of experience in various settings. She graduated with her BSN from Newman University and her MSN from Wichita State University. She previously worked as an ICU nurse and has been a board-certified Nurse Practitioner since 2014. She spent 10 years working alongside Dr. Chloe Steinshouer in pulmonary, sleep, and critical care medicine before starting her business. She is well known in the Wichita medical community and has connections to many local providers.
Rachael's goal for Kansas Care Connect is to provide personalized and thorough care to patients, while also filling gaps and healthcare needs within the local community.
Rachael and her husband have 4 children and 2 dogs. When not working, they enjoy attending kids' activities, traveling, and spending time with family and friends.
Regan graduated from Hutchinson Community College with a degree in Health Information Management. She also carries a certificate in medical coding.
Sarah has been in the healthcare field since 2007. She graduated from Wichita State University with her BSN and later from Maryville University with her MSN. She is a dual-boarded and has been working as a Nurse Practitioner since 2017 with experience in various settings and specialties.
Sarah knew from a young age that she was called to be in the healthcare field. When she transitioned from inpatient to outpatient care, the gaps, barriers, and communication issues related to patient care became grossly apparent. She is extremely excited to be a part of Kansas Care Connect, as she feels this is a service our community has been waiting for and will allow her to focus on her passion of advocating for patients to ensure they are provided with the care they deserve. She also wants to help patients feel empowered in managing and participating in their own care.
Courtney has worked in healthcare since 2010. She has worked in multiple care settings, including long term care, pre-op, and critical care. She has spent the last 10 years working as an ICU nurse.
Her passion is focusing on preventative care and connecting with her patients.
Melissa has been a nurse since 2015. Her specialty is critical care. As an ICU nurse, she knows how important it is for patients to have the tools they need to manage their health at home to avoid complications that lead to hospitalization. She’s passionate about advocating for her patients and customizing care to fit their needs and desires.
When Melissa isn’t at work, she’s a busy wife and mom of four children. She loves travel, music, and spending time with her family.
Sonya has been a nurse since 1989. She has developed skills across various specialties, including Pediatric Intensive Care, IT Nursing, and Infusion Nursing.
Her extensive career has been marked by a commitment to providing compassionate, high-quality care and a strong advocacy for patients to enhance their quality of life and improve health outcomes.
Alice brings extensive experience in complex and diverse
service delivery organizations. Her clinical specialty is the adult/geriatric population with a subspecialty in
palliative care.
Penny graduated from Wesley School of Nursing in 1980 and a BSN from Fort Hays University in 2007. Her nursing background includes ICU, ER, staff education, and Case Management - where she found a passion for patient education and coordinating community resources to improve health outcomes. She understands the importance of advocating for patients and their family as they navigate healthcare systems.
When not at work, she spends time with her family and training/competing with her dogs in various dog sports.
Bobby has been in healthcare going on 16 years. With a scholarship to play soccer after graduating high school, she obtained her Associate Degree in Science from Butler Community College. She then obtained CNA certification at the Wichita Technical Institute and began working at St. Francis hospital. She later enrolled at Cowley Community College and earned her EMT certification and graduated with her Associate Degree in Applied Science and Kansas/National MICT-Paramedic certification. As a working paramedic, her resume includes 7 years on an ambulance serving the citizens of Sedgwick County and 4 years as one of the founding paramedics in the ER at Wesley Medical Center.
Sadly, her beloved grandmother became ill with significant heart trouble. Rachael Rivero, a former high school classmate, and her team cared for and helped advocate for her care during that difficult time. The care and compassion provided by KCC was adamantly praised and other family members immediately signed up for services themselves. This connection led to her being hired to join KCC. A true family affair and she couldn’t be happier to be a part of this team!
Coach Nate is super pumped to join our Chronic Care Management team as our resident Health Coach.
Nate has a master's degree in Gerontology, a bachelor's degree in Exercise Science, and is both a Medical and Orthopedic Exercise Specialist. Nate is also a Functional Aging Specialist through the Functional Aging Institute, and Fitness Nutrition Specialist through the American Council of Exercise. As a Health Coach, Nate has been credentialed through the National Board for Health and Wellness Coaching since 2016. He is a winner of the Wichita Business Journal's Excellence in Healthcare award (2023), and a member of their Emerging Leaders class of 2023.
He is locally known as the "Aging Whisperer" and "Bringer of Joy."
CJ obtained his Doctor of Pharmacy degree from the University of Georgia College of Pharmacy in 2020. From there, he completed two years of Pharmacy Practice Residency at Wesley Medical Center. He is now a Board Certified Critical Care Pharmacist.
CJ believes that optimizing your care begins with your medication regimen and is dedicated to ensuring your medications work best for you.
Dr. Rivero has been practicing Internal Medicine since completing his residency in 2003. He works as a hospitalist in multiple facilities, but is primarily located at Wesley Medical Center in Wichita.
He is available as a medical consultant for our care team.
