As healthcare providers with years of experience working directly with patients in multiple care settings, it became apparent that there was a need for improved collaborative care services to help connect the dots in healthcare, especially for patients with chronic health conditions and complex needs. Kansas Care Connect exists to fill this need and help our community.  

Our team is dedicated to providing high-quality care management services to Kansas Medicare patients with chronic health conditions. 

We provide these services independently as well as through contracts with area physicians and medical groups. 

We do not replace your medical team. We simply work alongside them and serve as a resource to promote health and wellness.

Monthly service that focuses on health maintenance, education, and coordination of healthcare services. We create and maintain a personalized care plan as well as communicate with your entire healthcare team. Your dedicated care manager will check in regularly and assist with health education, medications, appointment scheduling, community resources, etc. We also offer health coaching with our certified health coach and a personalized pharmacist review as part of our program to ensure you meet your health goals.

Our experienced clinical team can monitor specific vitals as another way to track and manage your health. This allows for early interventions before a negative trend or change escalates. Data can be self-reported or sent electronically via provided devices that send readings as they are obtained. We can provide blood pressure cuffs, scales, blood glucose meters, pulse oximeters, peak flow meters, thermometers, and activity trackers. This service can be combined with our chronic care management program.
