OUR SERVICES

Chronic Care Management (CCM)

 We provide Medicare-covered Chronic Care Management services designed to help patients manage ongoing conditions while improving coordination with their healthcare providers.


Includes:

Comprehensive care plans

Monthly patient outreach

Medication review and coordination

Specialist and referral support

Connection to community resources

Education and advocacy

 

We work in collaboration with a patient’s primary care provider and specialists.

Remote Patient Monitoring (RPM)

RPM is a Medicare-covered program that allows care teams to track key health data between office visits using connected devices. RPM helps identify trends early, improve chronic condition management, and support timely intervention.


Includes:

  • Monitoring of vital signs 
  • Review of health data by qualified clinical staff
  • Patient education and support related to readings and trends
  • Communication with the patient’s healthcare team when concerns arise
     

*RPM is often used alongside CCM, but it is a separate program.

Advanced Care Planning (ACP)

 We support patients and families with thoughtful, guided planning for future healthcare decisions.


Includes:

  • Advance directive education and assistance
  • Goals-of-care conversations
  • Documentation coordination
  • Communication with healthcare providers

Supportive Services

 For patients already enrolled in our CCM program, we may provide acute care or short-term supportive clinical services when needed for safety and continuity.


These services:

Are temporary

Do not replace primary care

Are focused on transitions or gaps in access

Prioritize re-establishing long-term primary care

PeerLink® Programs

PeerLink® is our Medicaid program. These services are provided by contracted companies, Pinnacle Hope and Bridgepoint.

What Is Peer Support?

Peer support connects individuals with trained peers who use lived experience and structured support techniques to promote recovery, stability, and engagement. 

What is CPST?

Certified Psychiatric Supportive Treatment (CPST) is a structured, non-clinical service designed to help individuals build skills, stability, and engagement in daily life while managing behavioral health challenges.


CPST focuses on:

  • Developing coping and problem-solving skills
  • Improving emotional regulation and stress management
  • Strengthening daily functioning and independence
  • Addressing barriers related to social, environmental, and life stressors
     

CPST services are goal-oriented, skills-based, and supportive. They are not therapy and do not replace medical or mental health treatment. Instead, CPST complements clinical care by reinforcing strategies and skills in real-world settings.

Who PeerLink Serves

Medicaid patients with:

  • behavioral health needs
  • social or life transitions
  • long-term engagement needs beyond clinical visits

Who Monitors the PeerLink Programs?

The PeerLink® program is clinically supervised to ensure quality, safety, and alignment with best practices. These services are clinically supervised by a contracted Licensed Specialist Clinical Social Worker (LSCSW) and overseen by the APRN owner of Kansas Care Connect.
